Important: This form is for members and pledges of the Jax Bears only. Do not complete this form if you are not currently a member or a pledge.

1. Please provide your REAL NAME. This is NOT the name that will appear on the web site. This is only so the webmaster can match your name against the membership roster
2. Please provide an email address at which you can be contacted if there is a question about this form. Please be sure to include the @ sign and everything that appears after that (your domain name).
3. Please provide your name as you would like it to appear under your picture and in your bio. If you would like to use a nickname (such as Joe Joe Bear) instead of your real name, this is acceptable.
4. Please provide as much of the following as you are comfortable with: Date of Birth, relationship status, Occupation, IM contact info, home page address, email address for bio, and a short description of yourself. Review other bios for ideas.
